Description:
1. The subject of the order is the supply of medical devices, i.e. the supply of 137 automatic AED defibrillators for the Independent Public Health Care Facility of the District Emergency Service in Sosnowiec, in accordance with the assortment described in detail in Appendix No. 1A to this SWZ, which is also a detailed description of the subject of the order.2. In each item of the Assortment Form (Appendix No. 1A), the Contractor shall provide the manufacturer/manufacturer, trade name of the product and/or catalog number, if assigned in accordance with the requirements specified in the price form.3. In Appendix No. 1A to of this ToR, which is also a detailed description of the subject of the order, the minimum required technical and operational parameters of the subject of the order are described.4. The Ordering Party requires that the subject of the order be brand new, in complete condition, without the need for additional deliveries or incur costs and cannot be subject to third party rights or claims.5.
