Description:
1. The subject of the order is "Supply of single-use medical devices - repetition". A detailed description of the subject of the order is included in the contract template (with an annex), which is Annex No. 3 to the SWZ. The provisions of the SWZ should be read together with any changes to the content of the SWZ, e.g. as a result responses to Contractors' inquiries. 2. The Ordering Party allows the submission of partial offers. 3. The ordering party does not require the execution of the order by sheltered workshops, social cooperatives or other contractors covered by the provisions of Art. 94 section 1 of the Act of September 11, 2019 r. - Public Procurement Law (consolidated text: Journal of Laws of 2024, item 1320, as amended), hereinafter referred to as the Public Procurement Law. 4. The Ordering Party does not require the employment of persons on the basis of an employment relationship during the execution of the order (delivery). 5. The Ordering Party does not plan to conduct an electronic auction. 6. Ordering Party did not conduct preliminary market consultations before initiating the proceedings. 7. The Ordering Party, within the limits specified in Art. 462 et seq. the Act allows for the execution of the subject of the order - in each part of the order - by the Contractor using subcontractors. 8. Deadline binding of the offer until March 28, 2026. 9. The notification referred to in point 5.1.12 (Conditions of awarding the contract; Application conditions: Electronic application: Required) concerns the registration of the entity on the portal where the procedure is conducted. 10.
