• Description:
• 1. The subject of the order is the supply of single-use products, the detailed description, assortment, estimated quantities and requirements of which are specified in Appendix No. 5 to SWZ.2. Failure to meet even one of the requirements specified in Appendix 5 to the SWZ will result in: rejection of the offer. 3. The product range that is the subject of the order must be approved for trading and use in health care facilities in the Republic of Poland, in the case of the product listed as medical, in accordance with the Act of April 7, 2022 on medical devices (uniform text, Journal of Laws 2022, item 974) and be compliant with European Union law. Products must have a declaration of conformity and CE certificates for the offered medical device. 4. The offered products must be new, unused and required that the minimum shelf life of the offered products is half the period guaranteed by the manufacturer. In the case of a shorter date, prior consent of the Pharmacy Manager will be required.5. Delivery of disposable products will be based on orders honorable ones.
