Description:
1. The subject of the contract is the supply of one -time specialist medical devices for performing vascular procedures called "products"

Ordering party It requires the offer of medical devices - within the meaning of the Act of 7 April 2022 on medical devices (Journal of Laws 2022.974, as amended), hereinafter: "UOWM"), placed on the market and use on the territory of the Republic of Poland in accordance with applicable regulations.
Task No. 1
1. The subject of the order is the delivery of products specialized one -time medical use for performing vascular procedures, later called "products"

The contracting authority requires the offer of medical devices - within the meaning Of the Act of April 7, 2022 on medical devices (Journal of Laws 2022.974, as amended), hereinafter: "UOWM"), placed on the market and use on the territory of the Republic of Poland in accordance with applicable regulations.
4. The number of products given in the unit price form is indicative and the contracting authority has the right to order their smaller amounts, except that not less than 60% of the value of the contract.
5. The order consists of 15 tasks.
