1. Delivery of specialized medical products, including treatment kits and fluids for organ preservation
for transplantation, preparations for securing vascular catheters, hemostatic agents and
lenses for an ophthalmoscope for the needs of the USK-2 PUM Clinics in Szczecin.
2. The order consists of 15 tasks, the detailed description of which is included in Annex 2 of the FCJ.
3. The ordering party requires the offer of medical devices that have been introduced placed on the market in accordance with the Act of April 7, 2022 on medical devices (Journal of Laws 2024.1620, as amended), subject to letter a)
a) In the scope of task no. 2, item 7-10 in which the Ordering Party requires cosmetics to be offered (with a VAT rate of 23%)
Failure to meet the above condition will result in rejection offers.
4. The ordering party requires that the shelf life of all offered products is at least 12 months, counting from the date of delivery to the ordering party's headquarters.
5. The Ordering Party does not allow the submission of variant offers.
6. The contract in question is also suitable for small and medium-sized companies enterprises (SMEs)
