Description:
1) The subject of the contract is the supply of surgical and diagnostic gloves for the needs of the Provincial Hospital John Paul II in Bełchatow. A detailed description of the subject of the contract can be found in Annex 2 to SWZ.
2) The Ordering Party divided the subject of the order into 8 parts (packages from 1 to8), as described in chapter XXIII SWZ.
3) The subject of the contract offered in the procedure has all the necessary certificates, including declarations of compliance, is in accordance with the standards in force in the European Union, meets the requirements specified in the Act of April 7, 2022on medical devices and required by the superior regulations for trading and use.
4) Other requirements regarding the performance of the contract and the principles of cooperation between the Ordering Party and the Contractor specify the proposed provisions of the Public Procurement Agreement, constituting an attachmentNo. 3 to SWZ.
