Description:
1. The subject of the contract is: delivery of surgical aprons and napkins and sets for ophthalmological procedures hereinafter referred to as "productsFj), and the parameter form (Annex 2A to SWZ, hereinafter also as "FPW")
3. The contracting authority requires the offer of medical devices (in the scope of all tasks and positions) - within the meaning of the Act on medical devices (Journal of Laws 2024.1620, as amended), hereinafter: UOWM), placed on the market anduse in the territory of the Republic of Poland in accordance with applicable regulations.
4. Among the application, it requires that the shelf life for all products be used is a minimum of 24 months from the date of delivery to the headquarters of the ordering party.than 60% of the amount specified in the unit price form.
