The order has been divided into 23 packages.

1. In order to confirm the compliance of the offered supplies, the applicant requires the submission of the evidence listed below:
1) a statement that the offer offered has been approved and the introduced on the market in accordance with the provisions of the Act of 7 April 2022 on medical devices and has current declarations of compliance and certificates of notified unit- the content of the statement contained in the offer form, constituting Annex 1 to SWZ.
2) manufacturer's catalog cards and/or other manufacturer's materials confirming the parameters required in the description of the subject of the contract, containing the catalog numbers of the offered range and photos/drawings of the offered range.
3) exemplary samples of the offer offered for packages No. 10 and No. 23- 1 piece for each position in the package
2. The applicant requires that the evidence in question be prepared in electronic form and marked with a signature qualified by the Contractor.
