Description:
Delivery of ultrasound machine 1 for the PCM in Pleszew. 1. The subject of the order is the delivery of 1 piece of brand new, manufactured in 2025, ultrasound device with three heads for the Pleszew Medical Center in Pleszew. 2. The offered subject of the contract must have attestations, permits and certificates registration, certificates required by Polish law, on the basis of which it can be introduced to the market and used in health care facilities in the Republic of Poland. 3. The subject of the order must be a medical device that meets the requirements set out in Regulation 2017/745 of the European Parliament and of the Council of April 5 2017 on medical devices, amending Directive 2001/83/EC, Regulation (EC) No 178/2002 and Regulation (EC) No 1223/2009 and repealing Council Directives 90/385/EEC and 93/42/EEC (OJ EU L 2017 117 of 5 May 2017 , p. 1, as amended) and the National Medical Devices Act of April 7 2022, which applies to the application of this regulation, taking into account the transitional provisions (consolidated text: Journal of Laws of 2024, item 1620). 4.
