1. The subject of the order is the delivery, installation and commissioning of an ultrasound [ultrasound] device, hereinafter referred to as the ultrasound device or device for the Obstetrics and Gynecology Clinic of Miejskie Zespo Ochrony Zdrowotnej Sp. z o. o. in Wocawek, together with instructions and training in the use of the purchased equipment.2. The subject of the order includes: delivery of an ultrasound machine with equipment, its installation and commissioning at the location of the Obstetrics and Gynecology Clinic at ul. Kaliska 104a, 87-800 Wocawek at the Ordering Party's; providing instructions on the use of medical equipment referred to in point 1, in one, two or three dates, depending on the needs of the Ordering Party, after prior arrangement of the date of the instruction and the number of people with the Ordering Party.3. The device is to be delivered to the obstetrics and gynecology clinic located at: ul. Kaliska 104a, 87-800 Wocawek.4. The ordering party also requires full implementation, installation, configuration and integration of the delivered device with the Ordering Party's IT systems, as well as providing, as part of the contractual remuneration, appropriate licenses necessary for the correct transmission of data from the indicated appear...
