Description:
1. The subject of the order is the delivery of an ultrasound device as part of a subsidy from the Lower Silesian Voivodeship under the Civil Protection and Civil Defense Program. The order is financed from the budget of the Lower Silesian Voivodeship as part of a special-purpose subsidy granted for the purchase of an ultrasound device under the Civil Protection Program. and Civil Defense under Agreement No. 192/2025 of October 7, 2025. concluded between the Ordering Party and the Voivode of Lower Silesia.2. A detailed description of the subject of the order has been shown and described in the Price Form (Appendix No. 2), Parameter Card (Appendix No. 2a) and in the Draft Provisions agreement (Appendix No. 3) constituting an integral part of the Specification of Order Terms.3. The contractual conditions for the implementation of the public procurement in question include the draft provision of the contract, which is an integral part of the SWZ. The contractor undertakes to sign an agreement entrusting the processing of personal data with the conclusion of the contract (in accordance with Appendix No. 7 to the SWZ)
