Delivery of useful parts of medical apparatus - 5 task
task 1 - consumable filters compatible with the respirator Bennett
CD.
2) The contractor's statement regarding the condition described in art. 108 para. 1 point 5 PZP, i.e. a declaration of not belonging to the same capital group, within the meaning of the Actof February 16, 2007 on competition and consumer protection (Journal of Laws of 2020 item 1076, as amended), with another contractor who submitted a separate offer, partial offer or a statement of belonging to the same capital group together with documents or information confirming the preparation of the offer,partial offers regardless of another contractor belonging to the same capital group (according to Annex 3 to SWZ.);
3) information from the National Penal Register regarding the grounds for exclusion indicated in art. 108 para. 1 point 1.2 and 4 of the PZP Act, drawn up not earlier than 6 monthsbefore submitting
4) a copy or information from the National Court Register or from the Central Register and Information on Economic Activity *), in the scope of art. 109 para. 1 point 4 of the Public Procurement Law, drawn up not earlier than 3 months before its submission.
5) If the contractor consists of the resources of the entityproviding resources- documents regarding the entity providing resources- in order to demonstrate the lack of existence of the grounds for exclusion (to the extent similar to the contractor) and compliance (to the extent that the contractor cites his resources) conditions of participation inproceedings
