Description:
1. The subject of the contract is the supply of various drugs, cytostatic drugs, medical devices and foodstuffs for particular nutritional uses, listed in parts for the University Clinical Hospital of Fryderyk Chopin in Rzeszow.
2. Detailed description of the item withorder, the requirements of the Ordering Party are specified in the Price and Offer Forms - Attachments No. 1 for individual parts, which are an integral part of the ToR.items of the order in the quantities specified
in the Price and Offer Forms for the period:
- Part No. 1-4, 6-16, 20-25, 27-30, 32-39 - 12 months;
- Part No. November 9, 2023;
- Parts No. 18.19 - until January 31, 2024;
- Part No. 26 - until May 6, 2024;
- Part No. 31 - until September 6, 2024.
