Description:
1.1. The subject of the order is the supply of waste bags and clothing bags for a period of 24 months.
2. The detachment description of the subject of the contract is concluded by the price form constituting Annexes 1a to SWZ
The contracting authority requires the submission to confirm the compliance of the offered services The evidence listed below:
1) manufacturer's catalog cards and/or other manufacturer's source materials (e.g. folders,
Prospects, technical data or instructions for the range of offered) confirming the required parameters.
2) samples of sacks (for each item at least 3 pieces sacks welded together)
3. In a situation where the Contractor does not submit, together with the offer of documents indicated in point VIII.1 item 1) -2) or the submitted evidence in question is incomplete ordering contracting authority to submit them to submit or supplement them within the prescribed period.
1. The contractor is obliged to submit for the day submitting offers:
a) offer form using a pattern constituting Annex 1 to SWZ
b) Price form using a pattern constituting Annex 1A to SWZ.
c) the contractor's statement that it is not subject to exclusion - according to the template constituting Annex 2 to SWZ
d) in question The evidence referred to in chapter VIII SWZ
e) power of attorney (if concerned)
