1. The subject of the order is the design and execution of works aimed at the delivery and installation of a computed tomography device along with the reconstruction of the ZDO office premises into a computed tomography laboratory as part of a project implemented under the National Reconstruction Plan I Increasing Resilience - Component D "Efficiency, accessibility and quality of the health care system".
2. A detailed description of the subject of the order is included in Annex No. 1a to the SWZ, the assortment and price form constituting Annex No. 2 to the SWZ and in the required parameters and conditions - constituting Annex No. 2a to the SWZ.
3. The investment task will consist in: Designing and performing works aimed at "Delivery and installation of the device for Computed Tomography together with the reconstruction of the ZDO office premises into a tomograph laboratory. The detailed scope of tasks can be found in the PFU constituting Annex No. 3 to the SWZ.
4. The indicated rooms of the Diagnostic Imaging Department should be rebuilt computed tomography laboratory. The final stage is the delivery and assembly of the tomograph computer, ... - Documents:
