"Designing and performing works aimed at supplying and installing a device for tomography computer together with the reconstruction of the office rooms of the Imaging Diagnostic Plant into a computed tomography workshop as part of an project implemented from the National Plan of Reconstruction and Increasing Charges - Component D D "Efficiency, availability and quality of the healthcare system" (SPR: No. 17/25/ZP/PN)
1. The subject of the contract is the design and implementation of works aimed at the supply and installation of a computed tomography device together with the reconstruction of office rooms ZDO to the Computer Tomography Studio as part of the project implemented from the National Plan of Reconstruction and Increasing immunity - component D "Efficiency, availability and quality of the healthcare system".
2. The detachment description of the subject of the contract can be found in Annex 1a to SWZ, the assortment and price form constituting Annex 2 to SWZ and in the parameters and conditions required - constituting Annex No. 2a to SWZ.
3. The investment task will consist in: designing and performing works aimed at "delivery and assembly of the device to Computed tomography together with the reconstruction of the office rooms to the tomograph studio. "The detailed scope of tasks can be found in PFU constituting Annex 3 to SWZ.
4. The indicated rooms of the Imaging Diagnostics Department should be rebuilt Computer tomography studio. The last stage is delivery with the tomograph assembly Computer, ...
