Description:
services provided by the doctor will include activities in the field of: 1) ensuring a dignified life in illness and dying with dignity, treatment of pain in accordance with the indications of current medical knowledge using drugs available in the territory of the Republic of Poland, 2) treatment of other symptoms somatic diseases, prevention of complications, tests ordered by a doctor employed in the hospice, drug administration. 3) 24/7 access to services provided by a doctor will be provided. Two projects will be implemented for each person needing support medical advice per month4) The services will be supported by the use of remote channels for contact with the patient via telemedicine.5) Cooperation within the Therapeutic Team in terms of patient qualifications and creation of an Individual patient care plan (together with a nurse, a rehabilitator, a psychologist and the project administrator).6) The bidder provides its own transport to the patient.
