Description:
1. The subject of the contract is that the contractor (contractor) conducts financial statements of the medical entity (principal), drawn up in accordance with the provisions of the Accounting Act, for the following periods:
1) for the financial year ending on December 31, 2025;
2) for the financial year ending on December 31, 2026,
and submitting to the medical entity (client) a report on each of the examination, containing The opinion of an auditor about the examined financial statements or refusal to issue an opinion, in accordance with art. 83 para. 5 of the Act of May 11, 2017. o statutory auditors, audit companies and public supervision (Journal of Laws of 2024, item 1035), hereinafter referred to as the "Act on statutory auditors".
2. The order of the contract should be completed in accordance with the draft contract constituting Annex 1 to SWZ.
3. Design of the subject of the contract, including basic data economic and contact details regarding individual medical entities (clients), referred to in chapter 3 para. 2 point 1 lit. A-BB SWZ, contains Annex 2 to SWZ.
