The subject of the order is: "Expansion of the Care and Treatment Facility by creating infrastructure for chronically ill patients who will be provided with services aimed at maintaining vital functions" at ul. NadbrzeΕΌna 5a in Zgorzelec in the formula design and build, along with equipment, this Care and Treatment Facility (hereinafter also referred to as the Facility) as part of the National Reconstruction and Resilience Plan: Component D "Efficiency, accessibility and quality of the health care system" Investment D4.1.1 "Development of long-term care through "modernization of the infrastructure of medical entities at the district level" and renovation of the roof of the existing building using own funds.
The detailed scope of work and deliveries commissioned to the Contractor is specified in the Functional and Utility Program constituting Annex No. 7 to the SWZ, equipment specification and equipment constituting Annex No. 7.1 of the SWZ, the draft contract constituting Annex No. 8 to the SWZ as well as this specification of terms of contract and its annexes.
The task is carried out in the "design, implement" formula.
The Ordering Party requires and the Contractor undertakes to employ employees directly performing activities in the field of:
- related to the performance of demolition works,
- related to the performance of general construction works,
- related to the implementation of electrical and telecommunications installations,
- related to the construction of sanitary networks and installations
