- Numbering according to SWZ: 3.1. The subject of the order is: "Expansion of the Care and Treatment Facility by creating infrastructure for chronically ill patients who will be provided with services aimed at maintaining vital functions" with street NadbrzeΕΌna 5a in Zgorzelec in the design and build formula along with the equipment of this Care and Treatment Facility (hereinafter also referred to as the Facility) as part of the National Reconstruction and Resilience Plan: Component D "Efficiency, availability and quality health care system" Investment D4.1.1 "Development of long-term care through modernization of the infrastructure of medical entities at the district level" and renovation of the roof of the existing building using own funds. 3.2. Detailed scope of work and deliveries commissioned to the Contractor are specified in the Functional and Utility Program constituting Annex No. 7 to the SWZ, the specification of equipment and accessories constituting Annex No. 7.1 of the SWZ, the draft contract constituting Annex No. 8 to the SWZ as well as this specification of conditions order and its attachments. 3.3. The task is carried out in the "design, implement" formula. 3.6.
