1. The subject of the order is the expansion of the hospital HIS system with the module - Treatment paths within implementation of the task "Digitization of histopathological diagnostics, expansion of the hospital HIS system with the module - Treatment Paths and modernization of the infrastructure of the St. John of Dukla Oncology Center of the Lublin Region".
The Ordering Party did not divide the order into parts due to the fact that the order in question in technical, organizational and economic terms, it creates an inseparable whole. This decision will not distort competition by limiting the ability of smaller entities, in particular small and medium-sized enterprises, to compete for the contract. The subject of the order and its size correspond opportunities of entities from the SME sector. The decision was verified in terms of functionality and implementation methods used, and the structure of potential contractors on the market relevant to the subject of the contract was taken into account.
2. As part of the implementation of the subject of the order, the Contractor undertakes to:
a) Delivery and launch of application software modules
b) Granting a perpetual license for network use of the offered application software.
c) Integration of the offered software with systems owned by the Ordering Party.
d) Conducting training for users i administrators.
e) Covering the application software with warranty author's supervision for a period of 12 months.
3. The Contractor declares that:
