Description:
1. The subject of this order is the extension of the service contract regarding the ARIA system for the National Oncology Institute of Maria Skłodowska -Curie - National Research Institute of the Branch in Gliwice.
2. Description of the subject of the contract and the terms of the contract are included in:
1)Description of the subject of the contract - Annex 2 to SWZ,
2) of the planned provisions of the contract, which will be entered into the content of the contract - Annex 9 to SWZ.
3. The contracting authority does not allow the possibility of submitting partial offers.
4. Offers not containing the full scope of the subject of the contract will remainrejected.
5. The Employer allows the possibility of submitting equivalent offers - the equivalence description was included in Chapter IV SWZ.
6. The contracting authority requires the contractor to submit a bid bond and secure the proper performance of the contract.
7. The contracting authority does not provide for an electronic auction and orderssupplementary.
8. The Employer does not specify the requirements related to the implementation of the subject of the contract referred to in art. 95 para. 1 of the PZP Act.
9. Information clause resulting from the content of art. 13 of the Regulation of the European Parliament and of the Council (EU) 2016/679 of 27 April 2016 on protectionnatural persons in connection with the processing of personal data and on the free movement of such data and repealing Directive 95/46/EC, the contracting authority included in SWZ.
10. Information on the joint application of contractors for the award of the contract was included in Chapter IX SWZ.
