EZ.28.6.2024 Cleaning and disinfection service of air-conditioning and ventilation installations, including the replacement of HEPA filters in the facilities of the Provincial Multi-specialized Center for Oncology and Traumatology. M. Copernicus University in Łodź
1. The subject of the order is the insta cleaning and disinfection serviceair conditioning and ventilation systems, along with the replacement of HEPA filters in the facilities of the Provincial Multidisciplinary Center for Oncology and Traumatology. M. Kopernika in Łodź.
2. The entire service/subject of the order must be performed in accordance with the description of the subject of the order (hereinafter referred to as OPZ) constitutingAnnex No. 2 to the SWZ and the contract template (Annex No. 6 to the SWZ).
3. In the scope of items no. 1-16, 18-24 OPZ, the Contractor will commence the work after the date has been agreed by both parties by telephone.
4. Work related to implementation of the subject of the contract for items no. 1, 6, 7, 8, 13, 14, 18 of the OPZ may be inperformed only on the basis of the written consent of the Voivode of Łodź by temporarily suspending the medical activities of given departments.
5. Work related to the implementation of the subject of the contract for item 17 of the OPZ (PET Laboratory) may be performed only on the basis of the written consent of the radiological supervision inspectoron days when the cyclotron is not working, in accordance with the project for Radiological Protection of the Activities of the PET Laboratory with the Eclipse HP Cyclotron and the PET/CT device approved by the National Atomic Energy Agency.
