Description:
1. The subject of the order is the implementation of a comprehensive service provided to foster families in Maopolska, including children under their care, in the following counties: Nowy Targ, Suski and Tatra County within a period of up to 12 months from the date of conclusion of the contract, consisting in the provision of specialist services for a total maximum of 650 hours, for approximately 33 people (on average 20 hours per person) from foster families, including children under their care (their own or in foster care), hereinafter also referred to as project participants. The services will be provided on the basis and in accordance with the contract developed by the Ordering Party Individual Support Plan (IPW) based on an individual diagnosis of needs, which will be submitted to the Contractor within 5 days of its agreement with the project participant in the field of: - psychotherapy, - psychological, family, pedagogical and legal counseling - psychocorrection, psychoeducation, - mediation family, - psychological and psychotherapeutic assistance, - speech therapy/neurological therapy, - therapy for bonding disorders and experienced early childhood traumas and developmental disorders, - sensory integration.2.
