Description:
1. The subject of the contract is the purchase and supply of medicinal products - infusion fluids, clinical nutrition and hemofiltration fluids - 61 packages for the needs of the Katowice Oncology Center in the forms, doses and estimated quantities, whose list is divided into packages from 1 to 61 It constitutes Annex 2 to SWZ - price offer.
The subject of the contract is a medicinal product - an international name in the character and dose specified by the Employer.
2. Determination of the subject of the order in the common dictionary of orders:
33600000-6 Pharmaceutical products,
3. Requirements:
1) quantity the ordered medicinal product may be reduced, during the contract, depending on the current needs of the Employer related to the health services granted and will result from current orders without the possibility of pursuing claims by the Contractor for reducing the quantity purchased medicinal product.
2) The amount of ordered goods will not be less than 50 % of the contract value.
3) The medicinal product must be delivered in accordance with the provisions in force in this respect.
4) The contracting authority understands by the purchase and delivery of medicinal products to deliver to the hospital pharmacy implemented continuously at the request of the Ordering Party within the terms specified by him, i.e.
a) Cito - up to 24 hours
b) planned - up to 48 hours
from placing an order by the Ordering Party sent in writing via e -mail.
