Along with the application for admission to the proceedings, the template of which is Annex 1 to the GTC, the Contractor is obliged to submit the following documents:
125 para. 1 in joke. 395 para. 1 of the Act,

2) a power of attorney in particular to sign the application for admission
in proceedings, offers, subjective means of evidence, other documents
and confirming documents for compliance with the original, If they were signed by a proxy -if it concerns;
3) power of attorney (or other document from which the scope of authorization will result) to represent in the procedure of contractors jointly applying for the award of the contract - if concerned;
4) the obligation of the providing entity Resources to be given to the contractor with the necessary resources for the purposes of the contract in accordance with art. 118 para. 3 of the Act, the template is Annex 8 to the GTC - if it concerns.
5) a declaration of contractors jointly applying for the award of the contract, containing a list of services that They will be made by individual contractors, in accordance with art. 117 para. 4 in joke. 395 para. 1 of the Act - the template is Annex 7 to the GTC - if it concerns.
6) justification for the secret of the enterprise - if concerned.
