Description:
1. The subject of the order lease devices for automatic gram dyeing and dyeing in hematooncological tests together with the supply of compatible dyes for the needs of the USK-2 PUM laboratory diagnostics plant in Szczecin.
2. For the rest of the SWZ, the phrase will be used:
a) "products - this concept should be understood as reagents indicated in Annex 2 (FCJ) to SWZ;
b) "Equipment" - it is necessary to understand 2 leased devices whose descriptions and technical requirements include Annex 2 (FCJ).
3. The description of the subject of the contract was included in the unit price form (Annex 2 to SWZ, hereinafter also called "FCj") and should be submitted with the offer form (Annex 1 to SWZ, hereinafter also called "FO").
1, 2, 3, 6 and parts B item 1-3 and 5-7 as well as parts C were medical devices-within the meaning of the Act of 7 April 2022 on medical devices (Journal of Laws 20124.1620, as amended; hereinafter referred to as UOWM "), placed on the market and use on the territory of the Republic of Poland in accordance with applicable regulations.
b) in terms of parts A item 4 and 5 and part B item 4 Fathered chemical reagents
Failure to meet the above will result in the rejection of the offer.
