- 13. Subject of the order 13.1. The subject of this order is the implementation of social services in the form of long-term care for the residents of Krasnystaw in the period from the date of signing the contract until the completion of tasks under the Pomocna Do project - health services for people in need of support from the city of Krasnystaw, minimum 18 months. The implementation of long-term care means support in the form of long-term nursing care and care services for 25 project participants.13.2 Long-term nursing care will be provided for a period of 18 months from the date of conclusion of the contract by the nurses on average 24 clock hours per month for each person (25 people) (on average 75 clock hours per month for 1 nurse). The nurse position will be performed by 8 people. It is estimated that 10,800 hours of services will be provided under the contract. above support will be provided as part of organized visits at the place of residence of a given project participant from and dates adapted to the needs/potential/preferences of the participant (possibility of organizing visits in the afternoon/evening and on Saturdays and Sundays).
