Description:
1. Determining the subject of the contract:
The subject of the contract is the purchase of medical devices for cytometric diagnostics (diagnostic tests) in the quantities specified in the detailed description of the subject of the contract (hereinafter infected with HIV and patients with AIDS, together with storage service and shipping to laboratories performing tests for specialist centers/medical facilities selected through a competition for implementers, as part of the implementation of the government's health policy program: "Antitrats treatment people living with HIV in Poland for 2022-2026 "(hereinafter referred to as: health policy program) https://www.gov.pl/web/zdrowie/rzadowy-program-polityki-zdrowotna-leczenie-antiretrowiRiusowe-zzyjycych-z-wirusm-hiv-w-Polish
Each of the parts listed in OPZ is Subject of separate proceedings.
The order must be processed in full in accordance with the requirements set out in SWZ, OPZ and on the terms described in the planned provisions of the contract (hereinafter PPU), which constitute attachments to SWZ.
OPZ containing the requirements and conditions for the implementation of each of the Part of the order is an attachment to SWZ.
