1. The subject of this order is the modernization of the Prisma magnetic resonance imaging apparatus3T together with the implementation of adaptation works for the National Oncology Institute Maria Skłodowska-Curie-National Research Institute of the Branch in Gliwice, hereinafter referred to as the Employer.
2. Description of the subject of the contract and the terms of the contract are included in the attachments:
1) Technical parametersand assortment and price specification - Annex 2.1 - 2.2 to SWZ,
2) Designed contract provisions that will be entered into the content of the contract - Annex 4 to SWZ.
3) Description of the modernization of rooms - Annex No. 6 to SWZ,
4) IT infrastructure requirements - Annex No. 7 toSWZ,
5) Description of modernization of the ventilation, air conditioning and cooling system of magnetic resonance and technological installations - Annex 8 to SWZ.
3. Offers not containing the full scope of the subject of the contract will be rejected.
4. The Employer allows the possibility of submitting equivalent offers -The equivalence description was included in Chapter IV SWZ.
5. The contracting authority does not provide for an electronic auction and supplementary orders.
6. Information on the joint application of contractors for the award of the contract was included in Chapter IX SWZ.
7. The contracting authority provides for changesprovisions of the concluded contract (so -called contract changes based on Article 455 (1) point 1 of the Act) in relation to the content of the offer, on the basis of which the contractor was selected, in accordance with the conditions contained inAttack ...
