1. The subject of this order is the modernization of the Prisma 3T magnetic resonance imaging apparatus Along with the implementation of adaptation works for the National Oncology Institute Maria Skłodowska-Curie-National Research Institute of the Branch in Gliwice, hereinafter referred to as the Employer.
2. Description of the subject of the contract and the terms of the contract are included in the attachments:
1) Technical parameters and assortment and price specification - Annex 2.1 - 2.2 to SWZ,
2) the planned provisions of the contract, which will be entered into the content of the contract - Annex 4 to SWZ.
3) description of the modernization of rooms - Annex 6 to SWZ,
4) IT infrastructure requirements - Annex 7 to SWZ,
5) Description of the modernization of the ventilation system, air conditioning and cooling of magnetic resonance imaging and technological installations - Annex 8 to SWZ.
3. Offers not containing the full scope of the subject of the contract will be rejected.
4. The Ordering Party allows the possibility of submitting equivalent offers - description The equivalence was included in Chapter IV SWZ.
5. The Purchaser does not provide for an electronic auction and complementary orders.
6. Information on the joint application of contractors for the award of the contract was included in Chapter IX SWZ.
7. The contracting authority provides for the possibility of changes to the provisions the concluded contract (so -called contract changes based on Article 455 (1) point 1 of the Act) in relation to the content of the offer, on the basis of which the contractor was selected, in accordance with the conditions contained in Attack ...
