- Nursing services: 1) Specialized care for elderly, chronically ill people requiring specialized long-term care within the home hospice for 24 people per month for a period of 3 months each2) 24/7 access will be provided 7 days a week for services provided by a nurse.3) For one person needing support, 4 visits a week will be made depending on the current number of patients covered by home hospice services4) Services will be supported through the use of channels remote for contact with the patient, telemedicine.5) Cooperation within the therapeutic team in terms of patient qualifications and creation of an Individual patient care plan (together with a doctor, rehabilitator, psychologist and project administrator).6) The provider provides his own transport to the patient
