Description:
1. Subject of the order: service consisting in the provision of medical services in the field of labor law for 32 BLT, divided into parts:
Part 1 - provision of occupational medicine services for military units operating in Łask.
Part 2 provision of occupational health services to individuals military personnel operating in Sieradz.
The conditions and method of implementing the subject of the order are also specified in Annex No. 4 to the SWZ - contract template.
3.Benefits medical tests will be performed at the Contractor's medical facility:
For military units operating in Łask - at a distance of up to 60 km from Military Unit 1158 (ul. 9 Maja 95, 98-100 Łask) - in the communication line, measured on generally accessible public roads (according to commonly available navigation systems road)
For military units operating in Sieradz - within a distance of 60 km from Military Unit 1551 (ul. Wojska Polskiego 78, 98-200 Sieradz) - in the communication line, measured on generally accessible public roads (according to commonly available road navigation systems).
4. Type of services provided services/research is also specified in the price form, constituting Annex No. 3 to the SWZ, however, the number of tests in individual items of the form is not binding, these are forecasted amounts, serving only to evaluate and compare offers submitted in the procedure.
