Description:
1. The subject of this order are the transport of patients and blood, its components and biological materials for the National Oncology Institute. Maria Skłodowska -Curie - National Research Institute of the Branch in Gliwice.
2. Description of the subject of the contract and the conditions of implementationOrders are included in the following attachments:
1) Detailed conditions for the implementation of services in scope of tasks No. 1-2-Annexes 1.1-1.2 to SWZ,
2) assortment-price specifications-Annexes No. 3.1-3.2 to SWZ,
3) of the planned provisions of the contract that will be introduced toContent of the contract-Annexes No. 11.1-11.2 to SWZ.
3. The contracting authority allows the possibility of submitting partial offers - the subject of the contract is divided into 2 separate tasks, i.e. task 1 - patient sanitary transport services, task No. 2 - blood transport services, its components and materialsbiological.
4. The contracting authority allows the possibility of submitting offers for selected tasks.
5. Offers not containing the full scope of the subject of the contract will be rejected.
6. The Employer allows the possibility of submitting equivalent offers - the equivalence description was included in Chapter IV SWZ.
7. Insurance.The Contractor is obliged to conclude at his own expense the insurance contract:
- in terms of task No. 1:
 code of civil liability of the entity performing medical activities with a guarantee sum not lower than those resulting from the Regulation of the Minister of Finance of 29 April 2019 (Journal of Laws of 2019r...
