- 1. The subject of the order are patient sanitary transport services for the National Institute of Oncology. Maria Skłodowska-Curie - National Research Institute, Branch in Gliwice, hereinafter referred to as the Ordering Party.
2. Description of the subject of the order and terms of execution of the order are included in the following attachments:
1) Detailed conditions for the provision of services - Annex No. 1 to the SWZ,
2) Product and price specification - Annex No. 3 to the SWZ,
3) Draft provisions of the contract that will be introduced into the content of the contract - Annex No. 11 to the SWZ.
3. Offers that do not include full information scope of the subject of the order will be rejected.
4. The Ordering Party allows the possibility of submitting equivalent offers - the description of equivalence is included in Chapter IV of the SWZ.
5. The Ordering Party does not provide for electronic auctions or supplementary orders.
6. Information on applying together Contractors for the award of the contract are included in Chapter IX of the SWZ.
7. The Ordering Party provides for the possibility of changes to the provisions of the concluded contract (so-called contract changes based on Article 455(1)(1) of the Act) in relation to the content of the offer on the basis of which the Contractor was selected, in accordance with the conditions included in Annex No. 11 to the SWZ. The contract may also be changed in the cases referred to in Art. 455 section 1 points 2-4 and section 2 of the Public Procurement Law.
8. The Contractor is obliged to conclude insurance contracts at his own expense:
