Description:
The subject of the order is the provision of a repeatable service to the Ordering Party consisting in single-person duties as the Coordinator for the Organization of Health Protection (hereinafter referred to as KOOZ) in the Health Policy Office of the Capital City Office. Warsaw. Service KOOZ positions in the form of on-call duties will be performed continuously throughout the year, 24 hours a day, on all days of the week, including public holidays, in accordance with the established monthly duty schedule. Detailed scope of the order is specified in the template of the framework agreement with annexes, constituting Appendix No. 1 to the SWZ. The order will be implemented during the period of validity of the Framework Agreement, which is: 22 months from the date of conclusion of the Framework Agreement (but not earlier than February 1, 2026 r.), or until the amount referred to in from December 1, 2023 to November 30, 2025 is exhausted or until the implementation contracts for the total amount specified in section 2 are completed. 3 of the framework agreement template.
