The subject of the contract is: Service of collection, transport and utilization of waste with code 18 01 04
The contracting authority provides for the possibility of awarding supplementary contracts pursuant to art. 214 para. 1 point 7 - up to 30 % of the value of the basic order.
1. Exchange of declarations submitted with the offer:
1) declarationon meeting the conditions for participation in the procedure and demonstrating a lack of grounds for exclusion from the proceedings prepared in the form of a uniform European contract document (EAT) - according to the template constituting Annex 2 to SWZ.
