- The subject of the order is the design, construction, equipment and launch of the Nuclear Medicine Department. A detailed description of the subject of the contract is included in the following attachments to this SWZ:
1) Annex 1 to SWZ - scope of medical services of the Nuclear Medicine Department.
2) AnnexNo. 2 to SWZ-functional and utility program (PFU).
3) Annex 3 to SWZ-detailed description of the subject of the contract-medical devices.
4) Annex 3A to SWZ - detailed description of the subject of the contract - construction works.

The detailed scope of the subject of the contract is:
1) the design concept submitted to the Employer for approval,
2) complete construction design documentation, includingProject of permanent shields,
3) obtaining, based on project documentation, an administrative decision granting a building permit,
4) reconciliation of documentation, in the part concerning the project of permanent shields with the West Pomeranian State Provincial Sanitary Inspector,
5) Project documentationExecutive,
6) production, in the production plant of the building contractor in the form of modules with the highest possible finishing level,
7) transport of modules to the construction site at the Employer,
