- The subject of the order is property and civil liability insurance of the Provincial Ambulance and Sanitary Transport Station "Meditrans" SPZOZ in Warsaw. The description of the subject of the order is specified in detail in Annex No. 2 to the Terms of Reference. Performer undertakes to perform the subject of the order in accordance with all the requirements of the Ordering Party indicated in the ToR. The Ordering Party informs that if the offer is submitted by Towarzystwo Ubezpieczeń Wzajemnych, the Ordering Party will not become a member of TUW by concluding the contract. TUW submits an offer for terms and conditions consistent with the SWZ and its annexes, and any selection of the offer submitted by TUW will not be associated with acquiring or maintaining membership in TUW. All requirements specified in the order documents are minimum requirements and their fulfillment is obligatory. Failure to meet the above-mentioned minimum requirements will result in rejection of the offer as inconsistent with the terms of the contract pursuant to Art. 226 section 1 point 5 of the Public Procurement Law.
