Description:
1. The subject of the order is the provision of care services at the place of residence for the residents of the Municipal Social Welfare Center in Wejherowo.2. The subject of the order includes assistance in meeting everyday life needs, hygiene care, care recommended by a doctor and, if possible, ensuring contacts with the environment. Services are provided in accordance with Art. 17 section 1 point 11, art. 50 and art. 50a of the Act of March 12, 2004. on social assistance (i.e. Journal of Laws of 2025, item 1214, as amended) in the period from January 1, 2026 to December 31, 2026. The services are variable and depend on the individual needs of people entitled to the benefit and the current care situation.3. The amount of services expected during the contract implementation period: 1) The estimated amount of care services expected to be provided during the contract period is 6,004 hours per year; 2) The Ordering Party specifies the minimum number of hours to be completed during the term of the contract, at 4,000 hours per year.
