Description:
1. The subject of the order is the provision of care or neighborhood services at the place of residence (pursuant to Article 50 of the Act of March 12, 2004 on social assistance) for a maximum of 30 residents of the Stary Dzierzgo Commune, in accordance with the scope established in the administrative decision granting the benefit.2. Scope of services: including assistance in meeting everyday life needs, hygiene care, and ensuring contacts with the environment. People receiving support indicate the form of support they prefer in an individual needs diagnosis. The amount of support determined individually in an individual path3. Care services will be provided provided 7 days a week from 6:00 a.m. to 10:00 p.m. 4. The subject of this agreement is the provision of care services for a minimum of 30 beneficiaries, for a total of 6,600 hours over the entire duration of the agreement. 5. Detailed schedule and settlement of the number of hours in individual periods implementation of the contract will be determined after its conclusion, and the Contractor is obliged to ensure the provision of services for a total minimum amount of 6,600 hours.6.
