Description:
1. The subject of the order is to provide a comprehensive hotline service along with the provision of telecommunications services. As part of the comprehensive hotline service, customers will be scheduled for health services of the Profilaktyka 40 PLUS Program.
2. The stages of order implementation include:
Stage I: Prpreparation and launch of the service within 20 days from the date of conclusion of the contract.
Stage II: Implementation of the comprehensive service.
Stage III: Providing the Ordering Party with the data covering Stage III listed in point 2.10. OPZ.
3. When contacted by phone, the customer will have the opportunity to:
•ummaking an appointment for the provision of health care,
•filling out the survey for Profilaktyka 40 PLUS,
•changing and canceling the date of health care provision,
•obtaining information about the services provided,
•connecting to other hotlines indicated by the Ordering Party.
The service is another channelinformation, increasing accessibility for people who do not use the Online Patient Account or the MojeIKP application.
4. Additional goals are:
• increasing the availability of services for the client (by publicly providing information about reliable and available dates),
• increasing the use ofproviding resources in health care,
• increasing access to information,
• helping digitally excluded people complete the survey.
