Providing the concept and design documentation for the Central Pavilion building as part of the investment project entitled: "Construction of the Central Pavilion at the Dr. Anna Gostyńska Wolski Hospital sp. z o. o..... The subject of the order is: a.execution of the Multi-Sector Project Conceptual Design (hereinafter: WPK) together with the development, with the participation of the Ordering Party, of the investment evaluation form included in the Instrument for the Assessment of Investment Applications in the Health Sector (IOWISZ), the aim of which is to obtain a positive Objective Opinion Investment (OCI); b. preparation of Multi-Sectoral Design Documentation together with obtaining a final Building Permit Decision and providing technical consulting services during the public procurement procedure for the selection of the general contractor Investment Intent, including: - cooperation in the creation of the Terms of Reference, which will include detailed technical and formal requirements for the works; -answering substantive questions from potential contractors investment implementation area, including in particular: design and scope of work to be performed -support in assessing contractors' offers in terms of content, in particular verifying the compliance of the proposed solutions with the design and specification of conditions orders; -verification of the completeness and correctness of documents submitted by contractors in terms of formal and legal aspects c. provision of author's supervision services, in accordance with the Act of July 7, 1994 Construction Law (consolidated text: Journal of Laws 2025, items 418, 1080) in progress implementation of construction works in the scope of construction of the Investment Project entitled "Central Pavilion of the Wolski Hospital located at ul.
