Description:
1. The subject of the order is the provision of disposal services, including transport and utilization, of hazardous and medical waste and other waste in accordance with Appendix No. 2 to SWZ.2. The service should be performed in accordance with applicable regulations, in particular the provisions of the Act of December 14, 2012 on waste (consolidated text: Journal of Laws of 2023, item 1587, as amended) and the implementing provisions of this Act.3. Transport of waste from the Ordering Party's headquarters to the disposal site should be carried out using the Contractor's specialized means of transport, at the Contractor's expense.4. Waste disposal should be carried out in accordance with the regulations art. 20 of the Act of 14 December 2012 on waste. For the above circumstances, the Contractor should submit an appropriate document indicating the place of disposal of waste collected from the Ordering Party.5. The contractor is obliged to: 1) indicate the place of disposal (name of the entity, address, location incinerator), 2) indication of the alternative place of disposal (name of the entity, address, location of the incinerator), 3) confirms that it has the capacity to dispose of medical waste specified in the proceedings and in the quantities provided for in postpowa...
