Description:
1. The subject of the order is the provision of funeral services for the Municipal Family Assistance Center in Bytom as part of the implementation of the commune's own tasks of a mandatory nature pursuant to Art. 17 section 1 point 15 and art. 44 of the Act of 12 March 2004 on social assistance (consolidated text: Journal of Laws of 2025 item 1214 from Mon. d.). Funeral services concern the burial of children and adults, including the homeless, which will be provided in the form of cremation or in the traditional form.2. A detailed description of the subject of the order is included in Annex A to the specification (SWZ).3. The Ordering Party reserves the right to maintain prices unit, the right to change the forecast amount of services provided that are the subject of the order, depending on the specific types of burials necessary to perform. The rules of settlement for the implementation of the subject of the order are included in the Draft Contractual Provisions.4. The contractor must have: during the period of execution of the subject of the order, civil liability insurance in the scope of business activity.5.
