Description:
(Numbering according to SWZ) The subject of the order is the provision of funeral services commissioned by the Municipal Social Welfare Center in Łódź at municipal and religious cemeteries in the city of Łódź. 4.2 A detailed description of the subject of the order can be found here are included in Annex No. 1 and in the contract template Annex No. 5. The offer consists of the following documents: 15.1 Offer form - prepared according to the template - Appendix No. 2 to the SWZ. 15.2. Declaration of the Contractor regarding the grounds for exclusion under Art. 5k Regulation 833/2014 as amended by Regulation 2022/576 and Art. 7 section 1 of the Act on special solutions for counteracting support for aggression against Ukraine and for the protection of national security, submitted pursuant to Art. 125 section 1 of the Public Procurement Law, in accordance with the template constituting Annex No. 7 to the SWZ. In the case of contractors jointly applying for the contract, this declaration is submitted by each of these Contractors (also applies to the company's partners civil). 15.3 Power of attorney/powers of attorney for the person/persons signing the offer, if the offer is signed by an attorney (if applicable). 6. ORDER COMPLETION DEADLINE Order completion date: no earlier than January 2, 2026 December 31, 2026 or until the contract amount is exhausted, whichever comes first. 8.
