Providing health services by a doctor in the field of pediatric urology for the patients of the Institute "Monument - Children's Health Center"
1. Relialization of health services for IPCZD patients regarding qualifications for surgery, surgery and follow -up visits after surgery.
2. medical duty at the Pediatric Urology Clinic. The duty will be held on weekdays from 15.35 - 8.00 and on non -working days from 8.00 - 8.00.
3. The order includes 8 duty on weekdays and 3 duty on non -working days for 1 person.
4. The benefits will be granted in the clinic Pediatric urology and IPCZD operating theater.
5. In the celebrations, consultations and medical consultations to establish diagnostic and treatment plans.
6. The ordering order undertakes to keep medical records in accordance with the requirements set out in the law and applicable in IPCZD rules.
7. AMPREATION will be implemented by 1 person according to the monthly schedule for providing health services established between IPCZD and the recipient.
8. Duration of the contract: from the date of conclusion to 31.12.2027.
