- 1. The subject of the contract is to provide infusion fluids to the headquarters of the Ordering Party (Krakow, Os. Golden Autumn 1), as described and the requirements contained in Annex 1 to the NIN.
2. Surgery / warranty period of the subject of the contract: it cannot be less than 12 months from delivery dates,
And for product and oral nutrition products - shorter than 12 months, but not less than 6 months.
3. In drug programs and chemotherapy, only medicines placed on reimbursement lists of the National Health Fund are allowed (the price of the drug may not exceed the fixed financing limit by the National Health Fund).
4. The factory, by posting in the specification of the drug, e.g. "Table.", Means all types of tablets, "caps." Means all types of capsules, etc. In other cases, the form of the drug is exactly defined, i.e. "enteral table" means that only the tablet is allowed enteral.
5. A medicinal product, constituting the subject of the contract, should be understood as medicinal products within the meaning of the Pharmaceutical Law of September 6, 2001 (i.e., Journal of Laws of 2024 item 686, as amended). The offered medicinal product In art. 3 or 4 para. 1 and 2 or 4a of the Pharmaceutical Law.
6. The submitting, in the implementation of this order in the part regarding the transport of the medicinal product to the headquarters of the Ordering Party, shall allow the participation of subcontractors with permissions.
