Description:
1. The subject of the contract includes: 24 -hour provision of medical services in relation to detained persons, brought by officers from the reservation that they will be carried out for 7 days in
The contractor undertakes to provide the following services medical detained persons, brought by the officers of the ordering party: performing a medical examination of the person
detained meeting the conditions indicated in § 1 of the Regulation of the Minister of the Interior of 13 September 2012 on medical research of persons detained by the Police (Journal of Laws of 2012, item 1102) and provided for placing in a room for detainees, issuing a medical certificate of the lack or existence of medical contraindications to stay in a room for detainees with justification, as well as providing wounds and issuing prescriptions for necessary medicines.
2. The scope, size, quantity and requirements regarding the order were specified in the description of the subject of the contract hereinafter referred to as OPZ constituting Annex 7 to SWZ and in the price calculation forms, constituting for part of the order, Annex 2.1- 2.6 to SWZ.
3. Delivery requirements for the performance of the contract and payment of the subject of the contract have been specified the planned provisions of the contract, constituting, respectively, Annex 6.1 - 6.6 to SWZ.
