- 1. The subject of the order is the provision of medical services for employees of the company Katowickie Inwestycje S.A.. Scope of the order: 1) Basic order covering the provision of medical care, including in the field of occupational medicine, on the terms and conditions specified in the Description of the subject of the order, constituting an attachment No. 3 to the SWZ - for employees of Katowickie Inwestycje S.A.2) Order covered by the option right (if the Ordering Party exercises the option right) covering the provision of medical care on the terms and conditions specified in the Description of the subject of the order, constituting Appendix No. 3 to the SWZ - for members the family of an employee of Katowickie Inwestycje S.A. The optional order does not constitute an obligation on the part of the Ordering Party to award it, nor does it constitute a basis for the Contractor to pursue any claims for damages for failure to complete the order.2. A detailed description of the subject of the order can be found here in Annex No. 3 to the SWZ and in the draft provisions of the public procurement contract, which will be introduced into the contract (Annex No. 4 to SWZ).
